ABSTRACT

Este artigo partilha reflexões acerca da construção do estágio obrigatório curricular no campo da Psicologia Escolar/Educacional em uma perspectiva crítica. O estágio é um momento privilegiado da formação, por aproximar teoria e prática, em direção à práxis crítica, aspecto central nessa abordagem psicológica. Visando conhecer como tem sido construída a experiência de estágio e supervisão na área, foram realizadas entrevistas com um supervisor de estágio que atua em uma Universidade Pública do Piauí, dada a relevância de sua atuação e militância, buscando compreender: as concepções e práticas desenvolvidas pelo supervisor; os atravessamentos ético-políticos que emergem da experiência de estágio e supervisão; e os elementos de criticidade que marcam tal experiência. O artigo põe em foco as estratégias de organização do estágio, o papel da supervisão e os desafios vivenciados na relação estagiários-escola-supervisor. Esperamos contribuir com a reflexão acerca dos desafios implicados na construção de estágios supervisionados na área de Psicologia Escolar e Educacional em uma perspectiva crítica, tanto no campo, quanto nos encontros de supervisão.


Este artículo intercambia reflexiones acerca de la construcción de la pasantía obligatoria curricular en el campo de la Psicología Escolar/Educacional en una perspectiva crítica. La pasantía es un momento privilegiado de la formación, por acercar teoría y práctica, en dirección a la praxis crítica, aspecto central en ese abordaje psicológico. Visando conocer de qué forma ha sido construida la experiencia de pasantía y supervisión en el área, se realizaron entrevistas con un supervisor de pasantía que actúa en una Universidad Pública da Piauí, dada la relevancia de su actuación y militancia, buscando comprender: las concepciones y prácticas desarrolladas por el supervisor; los atravesamientos ético-políticos que surgen de la experiencia de pasantía y supervisión; y los elementos de criticidad que marcan tal experiencia. El artículo pone de relieve las estrategias de organización de la pasantía, el papel de la supervisión y los desafíos vivenciados en la relación alumno en formación-escuela-supervisor. Esperamos contribuir con la reflexión acerca de los desafíos implicados en la construcción de pasantías tuteladas en el área de Psicología Escolar y Educacional en una perspectiva crítica, en el campo y en los encuentros de supervisión.


This article shares reflections about the construction of mandatory curricular internship in the field of School and Educational Psychology from a critical perspective. The internship is a privileged moment in training, as it brings theory and practice closer together, towards critical praxis, a central aspect of this psychological approach. In order to know how the experience of internship and supervision in the area has been constructed, interviews were conducted with an internship supervisor who works at a Public University of Piauí, given the relevance of his work and militancy, seeking to understand: the concepts and practices developed by the supervisor; the ethical-political crossings that emerge from the internship and supervision experience; and the elements of criticality that mark such an experience. The article focuses on internship organization strategies, the role of supervision and the challenges experienced in the relationship between interns-school-supervisor. We hope to contribute to reflection on the challenges involved in building supervised internships in the area of School and Educational Psychology from a critical perspective, both in the field and in supervisory meetings.

ABSTRACT Objective: To analyze the compliance with the commercialization of children's products included in the Brazilian Code of Marketing of Infant and Toddlers Food and Childcare-Related Products (NBCAL) in drugstores in Uberlândia/MG. Methods: A cross-sectional study was carried out in 143 drugstores that sold infant products: infant formula (IF), follow-up IF, nipples, teats, pacifiers and nipple shields; FI for young children, transition foods and cereal-based foods, fluid or powdered milk, modified/similar milks of plant origin and dairy compounds. The location of drugstores in the five geographic sectors was performed by geoprocessing. The data collected were: types of promotion and types of drugstore administration (drugstore chains/drugstores with independent administration). Irregular commercial promotion was expressed as absolute and relative frequencies. Results: Irregular commercial promotion was found in 11.7% of nipples, pacifiers and bottles, in 10.0% of IF and follow-up formula, in 9.5% of IF for young children, in 11.1% fluid or powdered milk, in 25.0% of transition foods and cereal-based foods and in 59.1% of dairy compounds. In commercial drugstore chains, the presence of promotion for dairy (81.8 vs. 28.6%, respectively) was higher than in drugstores with independent administration. The opposite ocurred for fluid or powdered milk, modified and similar milks of plant origin. The downtown and eastern sectors had the highest percentages of promotions (26%). Conclusions: NBCAL violations still occur in drugstores, mainly in the sale of young children's foods and in the commercial network drugstores.


RESUMO Objetivo: Analisar a conformidade da comercialização dos produtos infantis incluídos na Norma Brasileira de Comercialização de Alimentos para Lactentes e Crianças de Primeira Infância, Bicos, Chupetas e Mamadeira (NBCAL) e de compostos lácteos em drogarias de Uberlândia/MG. Métodos: Estudo transversal realizado em 143 drogarias que vendiam produtos infantis: fórmulas infantis (FI) para lactentes, FI de seguimento para lactentes, mamadeiras, bicos, chupetas e protetores de mamilo; FI para crianças de primeira infância, alimentos de transição e alimentos à base de cereais, leites fluidos/em pó, leites modificados/similares de origem vegetal e composto lácteo. A localização das drogarias nos cinco setores geográficos foi realizada por geoprocessamento. Os dados coletados foram: tipos de promoção comercial irregular e tipo de administração da drogaria (rede/independente). As promoções comerciais irregulares foram expressas em frequências absoluta e relativa. Resultados: Verificamos a presença de promoção comercial irregular em 11,7% dos bicos, chupetas e mamadeiras, em 10,0% das FI lactentes/seguimento de lactentes, em 9,5% das FI para crianças de primeira infância, em 11,1% dos leites, em 25,0% de alimentos de transição e em 59,1% dos compostos lácteos. Nas drogarias de rede, a presença de promoção comercial irregular foi maior para compostos lácteos (81,8 vs. 28,6%, respectivamente) e, para leites, foi maior nas drogarias independentes (30,8 vs. 6,0%). Os setores central e leste apresentaram os maiores percentuais de promoção comercial irregular (26%). Conclusões: As violações à NBCAL ainda ocorrem nas drogarias, principalmente para os produtos destinados às crianças de primeira infância, e nas drogarias de rede.

ABSTRACT

On-site supervision is a risk-based regulatory system that requires the scientific development of supervision plans for quality risks and hidden dangers in pharmaceutical enterprises, the rational allocation of supervision resources based on their risk levels, and the implementation of classified supervision measures. In this study, the quality risk monitoring business support system is set up for pharmaceutical enterprises by establishing the quality risk expert database and quality risk monitoring index system for pharmaceutical enterprises based on the difficulty analysis of on-site drug supervision. Based on this support system, the quality risk classification method, the differentiated spot check strategy and business auxiliary visualization system are established. This support system is used to learn the risk level of pharmaceutical enterprises, so as to innovate supervision methods and optimize monitoring strategies. Taking Jiangxi Province as an example, it is verified that the support system can guide the risk assessment of sample enterprises, can improve the targeting of on-site drug supervision in the process of technical review, scheme editing, on-site implementation and comprehensive evaluation, and can effectively improve the quality and efficiency of supervision.

ABSTRACT

El presente trabajo se centra en un estudio preliminar en aras de aplicar el Algoritmo David Liberman (ADL) como instrumento en el campo de la supervisión en un texto, tomado como informe de un supervisante, que pretende tratar el problema relacionado con el cambio psíquico desde las intervenciones del analista y desde una dificultad que surge con el uso que hace el paciente de la intervención para atacar al análisis. Comienza con un estudio de un enactment contratransferencial relatado por el analista, y luego analiza la construcción teórica presentada por él, identificando que su elaboración conceptual aún era inducida por el influjo del discurso del paciente. El énfasis de la investigación propuesta es aplicar la taxonomía de las intervenciones junto a la perspectiva de los errores del pensamiento aportadas por Maldavsky (2017). Se propone articular la práctica clínica y la elaboración conceptual, tal como ocurre en la supervisión AU


The present work presents the possibility of applying the David Liberman Algorithm (ADL) as an instrument in the field of supervision, from a text taken as a report by a supervisor, which aims to address the problem related to psychic change from the analyst's interventions and of a difficulty that arises from the patient's use of the intervention to attackthe analysis. It begins with a study of a countertransferential enactmentreported by the analyst, and later analyzes the theoretical construction presented by him, identifying that his conceptual elaboration was still induced by the influx of the patient's speech. Introducción: Las supervisiones clínicas cumplen un rol esencial dentro de la formación profesional del psicólogo clínico. Se han desarrollado diferentes modelos de supervisión, sin embargo, pocos estudios abordan el cómo se desarrolla el proceso de supervisión propiamente tal. Objetivo: Describir las dinámicas de supervisión clínica grupal en la Unidad de Adultos del Servicio de Psicología Integral de una Clínica Universitaria, que proporciona atención psicológica a la comunidad. Metodología: Estudio cualitativo descriptivo y de alcance transversal, en una muestra de 5 sesiones de supervisión videograbadas, cuya interacción verbal fue sometida a análisis de contenido convencional. Resultados: Se distinguen aspectos de la apertura del proceso de supervisión, en el cual se identifica una dificultad variable de los supervisados para formular preguntas de supervisión, dificultad enmarcada en un contexto de involucramiento afectivo de los supervisados. Se identifican intervenciones de los miembros del equipo de supervisión: una transversal de validación de la experiencia del supervisado; e intervenciones durante el proceso de supervisión, como, por ejemplo, preguntas dirigidas a revisar "la experiencia" del supervisado durante la atención del caso; "construcción de hipótesis comprensivas del caso", y la entrega de "sugerencias para el abordaje terapéutico" del caso en específico, y/o que pueden ser aplicados a otros casos.


Background: Clinical supervision plays an essential role in the professional training of clinical psychologists. Different supervision models have been developed; however, few studies address how the ongoing process of supervision is developed. Objective: To describe the dynamics of clinical group supervision at the Adult Unit of the Clinical Psychology Service of a University Clinic, which provides psychological care to the community. Methodology: A descriptive qualitative study of cross-sectional scope, in a sample of 5 video-recorded supervision sessions, whose verbal interaction was subjected to conventional content analysis. Results: Aspects of the opening of the supervision process are distinguished, in which a different degree of difficulties of the supervisees to formulate supervision questions is identified, a difficulty framed in a context of affective involvement of the supervisees. Interventions by members of the supervision team are identified: a validation of the supervisee's experience which is present each one supervision process; and specific interventions during the supervision process, for example, questions aimed at reviewing «the experience» of the supervisee during the care of the case; «construction of comprehensive hypotheses of the case», and the delivery of «suggestions for the therapeutic approach» of the specific case, and/or that can be applied to other ones.

Objetivo: analisar a relação entre o contexto de trabalho das lideranças de enfermagem na pandemia da COVID-19 e o Burnout. Método: estudo de método misto, com uma amostra quantitativa de 64 líderes de enfermagem, de todos os setores de quatro hospitais gaúchos. A coleta de dados ocorreu de agosto a outubro de 2020, após aprovação do Comitê Nacional de Ética em Pesquisa. Foi realizada a análise inferencial dos dados quantitativos (n=64), aplicados os testes de Mann-Whitney e correlações bivariadas de Spearman, considerando diferenças estatisticamente significativas "p" bicaudal menor que 0,05. Realizaram-se entrevistas qualitativas (n=12) acerca dos impactos da pandemia. Resultados: identificou-se 6,3% de prevalência de Síndrome de Burnout. Houve associação entre os domínios Desgaste Emocional e Despersonalização e as variáveis medo, aumento do consumo de álcool e impacto na saúde (p<0,05). Evidenciaram-se mudanças no contexto laboral, aumento do nível de exigência e da carga de trabalho e impactos na saúde. Conclusão: conclui-se que há associação entre o contexto de trabalho na pandemia e o Burnout. O aumento da sobrecarga de trabalho repercutiu em prejuízos na saúde mental.


Objective: to analyze the relationship between the work context of nursing leaders in the COVID-19 pandemic and Burnout. Method: mixed method study, with a quantitative sample of 64 nursing leaders, from all sectors of four hospitals in Rio Grande do Sul. Data collection carried out from August to October 2020, after approval by the National Research Ethics Committee. Inferential analysis of quantitative data (n=64) was performed, applying Mann-Whitney tests and bivariate Spearman correlations, considering statistically significant two-tailed "p" differences less than 0.05. Qualitative interviews (n=12) were carried out about the impacts of the pandemic. Results: a 6.3% prevalence of Burnout Syndrome was identified. There was an association between the Emotional Exhaustion and Depersonalization domains and the variables fear, increased alcohol consumption and impact on health (p<0.05). There were changes in the work context, an increase in the level of demand and workload, and impacts on health. Conclusion: it is concluded that there is an association between the work context in the pandemic and Burnout. The increase in work overload had negative effects on mental health.

Se realizaron entrevistas cualitativas (n=12) sobre los impactos de la pandemia. Resultados: se identificó una prevalencia del Síndrome de Burnout del 6,3%. Hubo asociación entre los dominios Cansancio Emocional y Despersonalización y las variables miedo, aumento del consumo de alcohol e impacto en la salud (p<0,05). Se han producido cambios en el contexto laboral, aumento en el nivel de exigencia y en la carga de trabajo e impactos en la salud. Conclusión: se concluye que existe asociación entre el contexto laboral en la pandemia y el Burnout. El aumento de la sobrecarga de trabajo tuvo efectos negativos sobre la salud mental.

La supervisión de los servicios de enfermería es fundamental para constatar que se proporcione el cuidado con eficiencia, eficacia, calidad, calidez y equidad a la persona familia y comunidad, con conocimientos técnico científicos y tecnológicos, en las Redes Integrales e Integradas de Salud (RIIS) del Ministerio de Salud. En ese sentido los(as) profesionales de enfermería, aportan decididamente a la organización de la prestación de servicios de salud, ajustando su accionar al enfoque integral; por ello es importante fortalecer la función de supervisión en los diferentes niveles de atención para incidir positivamente, con procesos y procedimientos técnico-administrativos que faciliten la mejora continua de la calidad del cuidado de enfermería, que forma parte de un equipo multidisciplinario dentro de la organización


Supervision of nursing services is essential to ensure that care is provided efficiently, effectively, quality, warmth and equity to the family and community, with scientific and technological know-how, Integrated Health Networks (RIIS) of the Ministry of Health. In this sense, the nursing professionals(s) contribute decisively to the organization of the provision of health services, adjusting their actions to the comprehensive approach; therefore it is important to strengthen the supervisory function at the different levels of care in order to have a positive impact, with processes and administrative procedures that facilitate the continuous improvement of the quality of nursing care, part of a multidisciplinary team within the organization

ABSTRACT

Resumo Este estudo analisa as situações de trabalho vividas pelos profissionais na fiscalização sanitária de medicamentos da Agência Nacional de Vigilância Sanitária, durante a pandemia de COVID-19. Estudo de caso único, de natureza qualitativa, com abordagem ergológica. Os dados foram produzidos entre 2020 e 2021, por meio de 18 entrevistas e observação participante em três reuniões de trabalho online. As práticas discursivas foram sistematizadas em mapas ergodialógicos, resultando em três categorias: os trabalhos vividos na fiscalização dos medicamentos; as variabilidades do contexto organizacional e as renormalizações frente à pandemia. As fragilidades dos sistemas de informação, o marco regulatório desatualizado, o modelo de atuação, a fragmentação do trabalho, dentre outros, ocasionaram entraves à realização da atividade pelo profissional. A conformação de novo modo de fazer fiscalização envolve a combinação de intervenções direcionadas à prevenção, detecção e resposta ao consumo de medicamentos nocivos à saúde. A transformação necessária ultrapassa a função restrita de controle e punição e se consolida como um novo modelo de atuação proativo e dinâmico, baseado na gestão do risco sanitário e na implantação das Boas Práticas de Fiscalização.


Abstract This study analyzes the work situations experienced by professionals in the sanitary fiscalization of medicines at the Brazilian Health Regulatory Agency, during the COVID-19 pandemic. This is a single-case study, qualitative in nature, with an ergological approach. The data were collected from 2020 to 2021. During this period, 18 interviews and 3 online work meetings were conducted. The discursive practices were systematized in ergodialogical maps, resulting in three categories: the works experienced in the sanitary enforcement of medicines; the variabilities of the organizational context and the renormalizations facing the pandemic. The fragilities of the information systems, the outdated regulatory framework, the performance model, the fragmentation of the work, among others, have caused barriers to the professional's activity. The conformation of a new way of performing the sanitary fiscalization of medicines involves the combination of interventions aimed at prevention, detection, and response to the consumption of medicines that are harmful to health. The necessary transformation goes beyond the restricted function of control and punishment and consolidates itself as a new proactive and dynamic action model, based on health risk management and the implementation of Good Fiscalization Practices.

ABSTRACT

presentamos aqui um modelo de supervisão clínica horizontal, denominado Intervisão, e procuramos demonstrar sua relevância no suporte emocional e aprimoramento das condições de trabalho de duas coordenadoras de Grupo Operativo, ao longo de uma pesquisa-ação de seis meses de duração. A Intervisão é apresentada em sua história conceitual e nas formas como vem sendo aplicada nos campos da saúde, da educação e da formação de profissionais, bem como no seu paralelismo no campo da formação permanente de psicanalistas. Descrevemos a Intervisão realizada por essa equipe de sete grupanalistas acompanhando o trabalho de duas de suas integrantes, coordenadoras de dois Grupos Operativos com profissionais dos CAPS do estado de São Paulo. Este recurso ofereceu contenção emocional, ampliou as perspectivas e aprimorou as habilidades que deram confiança às coordenadoras e aos membros da equipe. Trata-se de um recurso que ajuda a incorporar a prática reflexiva como proposta para a metodologia de estudo utilizada. O ineditismo está na utilização da Intervisão como recurso metodológico numa pesquisa qualitativa, garantindo análise cuidadosa do processo de intervenção, como também de uma produção de conhecimentos gerada a partir do saber e do fazer grupal, integrando teoria e prática.


We present here a model of horizontal clinical supervision, called Intervision, trying to demonstrate its relevance in the emotional support and improvement of the working conditions of two Operative Group coordinators, throughout a six-month action-research. Intervision is presented in its conceptual history and in the ways it has been applied in the fields of health, education and professional training, as well as in its parallelism in the field of the permanent training of psychoanalysts. We describe the Intervision carried out by this team of seven group analysts following the work of two of its members, coordinators of two operative groups with CAPS professionals in the state of São Paulo. This resource provided emotional restraint, broadened perspectives, and honed skills that gave coordinators and team members confidence. It is a resource that helps to incorporate reflective practice as a proposal for the methodology of study used. What is unique is the use of Intervision as a methodological resource in qualitative research, ensuring careful analysis of the intervention process as well as the production of knowledge generated from group knowledge and practice, integrating theory and practice.


Presentamos aquí un modelo de supervisión clínica horizontal, denominado Intervision, y tratamos de demostrar su relevancia en el apoyo emocional y en la mejora de las condiciones de trabajo de dos coordinadores de Grupo Operativo, a lo largo de una investigación-acción de seis meses. Se presenta la Intervisión en su historia conceptual y en las formas en que ha sido aplicada en los campos de la salud, la educación y la formación profesional, así como en su paralelismo en el campo de la formación permanente de psicoanalistas. Describimos la Intervisión realizada por este equipo de siete analistas de grupo siguiendo el trabajo de dos de sus miembros, coordinadores de dos grupos operativos con profesionales de CAPS en el estado de São Paulo. Este recurso proporcionó contención emocional, perspectivas ampliadas y habilidades perfeccionadas que dieron confianza a los coordinadores y miembros del equipo. Es un recurso que ayuda a incorporar la práctica reflexiva como propuesta para la metodología de estudio utilizada. Lo que es distinto es el uso de Intervisión como recurso metodológico en la investigación cualitativa, asegurando un análisis cuidadoso del proceso de intervención, así como la producción de conocimiento generado a partir del saber y hacer grupal, integrando la teoría y la práctica.

RESUMO: Objetivo: comparar índices e indicadores de qualidade dos cuidados de enfermagem nos serviços de internação, médico e cirúrgico quando implementada a supervisão clínica. Método: estudo observacional, retrospectivo com abordagem quantitativa, e amostragem do tipo aleatória simples dos registros das auditorias relacionados à qualidade dos cuidados de enfermagem (n=719). A pesquisa decorreu entre agosto de 2020 e agosto de 2022, em dois hospitais privados da região de Lisboa, Portugal, onde se realizam auditorias relacionadas à qualidade dos cuidados de enfermagem, sendo implementada a supervisão clínica num dos hospitais. Empregou-se estatística descritiva e inferencial. Resultados: decorrente da análise dos registos quando comparados os hospitais e serviços foi evidenciada subida de scores de qualidade desejada e adequada (≤5 e ≤4) nos serviços do hospital onde foi implementada a supervisão clínica. Conclusão: a supervisão clínica permite acompanhar estrategicamente as equipes na prática dos cuidados, elevando os índices e indicadores de qualidade com impacto direto para o paciente.


ABSTRACT Objective: to compare indices and indicators of the quality of nursing care in inpatient, medical and surgical services when clinical supervision is implemented. Method: an observational, retrospective study with a quantitative approach and simple random sampling of audit records related to the quality of nursing care (n=719). The research took place between August 2020 and August 2022, in two private hospitals in the Lisbon region, Portugal, where audits related to the quality of nursing care were carried out, and clinical supervision was implemented in one of the hospitals. Descriptive and inferential statistics were used. Results: analysis of the records when comparing the hospitals and services showed an increase in the desired and adequate quality scores (≤5 and ≤4) in the services of the hospital where clinical supervision was implemented. Conclusion: Clinical supervision allows teams to be strategically monitored in the practice of care, increasing quality indices and indicators with a direct impact on the patient.


RESUMEN Objetivo: comparar los índices e indicadores de calidad de la atención de enfermería en los servicios de hospitalización, médicos y quirúrgicos cuando se aplica la supervisión clínica. Método: estudio observacional, retrospectivo, con abordaje cuantitativo y muestreo aleatorio simple de registros de auditoría relacionados con la calidad de los cuidados de enfermería (n=719). La investigación tuvo lugar entre agosto de 2020 y agosto de 2022 en dos hospitales privados de la región de Lisboa, Portugal, donde se realizan auditorías relacionadas con la calidad de los cuidados de enfermería, y en uno de los hospitales se implementó la supervisión clínica. Se utilizó estadística descriptiva e inferencial. Resultados: el análisis de los registros al comparar los hospitales y servicios mostró un aumento de las puntuaciones de calidad deseada y adecuada (≤5 y ≤4) en los servicios del hospital donde se implantó la supervisión clínica. Conclusión: La supervisión clínica permite el seguimiento estratégico de los equipos en la práctica asistencial, aumentando los índices e indicadores de calidad con impacto directo en el paciente.

Introducción:La supervisión de enfermería juega un papel fundamental en la implementación de estrategias de aprendizaje dirigido que mejoren el desempeño laboral de los profesionales, garantizando así una atención de calidad. Objetivo:Evaluarla relación que existe entre la supervisión a través del aprendizaje dirigido y el desempeño laboral de enfermería. Metodología: estudio cuantitativo con diseño descriptivo, correlacional de corte transversal desarrollado en el área de urgencias deun Instituto Nacional de Salud de la Ciudad de México, en2022. El universo estuvo conformado por profesionales de enfermería del Instituto Nacional deNutrición, la muestra fue de 90profesionales seleccionados por muestreo aleatorio simple, Se utilizó un instrumento que mide el desempeño laboral y la supervisión a través de una escala tipo Likert que va de nunca a siempre, clasificando las variables en tres categorías: nivel bajo, nivel medio y nivel alto. Resultados:existe una correlación directamente proporcional entre la supervisión a través del aprendizaje dirigido y el desempeño laboral, es decir un nivel medio de la categoría del instrumento, a mayor supervisión mayor desempeño laboral; así como, cuando existe mayor capacitación, comunicación, supervisión programada, hay un mayor desempeño laboral. Conclusiones: Los resultados evidencian que al impulsar la implementación del proceso de supervisión mediante el modelo de aprendizaje dirigido es posible potencializar el desempeño laboral de los profesionales de enfermería. Asimismo, el desempeño laboral del profesional se beneficia mediante las estrategias de supervisión como son: la capacitación continua, la comunicación efectiva, la retroalimentación y la supervisión programada


Introduction:Nursing supervision plays a fundamental role in the implementation of directed learning strategies that improve the job performance of professionals, thus ensuring quality care. Objective: To evaluatethe relationship between supervision through directedlearning and nursing job performance. Methodology: quantitative study with a descriptive, correlational, cross-sectional, cross-sectional design in the emergency department of a National Health Institute in Mexico City, from March to May 2022. The universe was made up of nursing professionals from the National Instituteof Nutrition, the sample was 90professionals selected by simple random sampling. An instrument was used to measure job performance and supervision through a Likert-type scale ranging fromnever to always, classifying the variables into three categories: low level, medium level and high level. Results: there is a directly proportional correlation between supervision through directed learning and job performance, i.e. a medium level of the instrument category, the higher the supervision the higher the job performance; as well as, when there is more training, communication, programmed supervision, there is higher job performance. Conclusions: The results show that by promoting the implementation of the supervision process through the directed learning model, it is possible to enhance the work performance of nursing professionals. Likewise, the professional's work performance benefits from supervision strategies such as: continuous training, effective communication, feedback and programmed supervision

@#Abstract: Objective To retrospectively analyze the individual dose monitoring of radiation workers in Chongqing City from 2016 to 2020, so as to provide scientific reference for occupational health monitoring and radiation protection. Methods Radiation workers who performed individual dose monitoring in Chongqing Center for Disease Control and Prevention from 2016 to 2020 were selected as the research objects, and the monitoring data were collected and analyzed. Results A total of 40 926 individuals were included in this study, with a total of 135 622 person-years. The average annual individual effective dose of radiation workers over the five-year period was 0.56 mSv. NR1 and NR5, the ratio of the number of works receiving annual individual does exceeding 1 and 5 mSv to the to the total monitor workers were 13.019% and 0.335%. The annual effective dose per capita in Chongqing City showed a trend of first decreasing, then increasing and then decreasing again over the study period, with significant differences between different years (F=971.754, P<0.001). Although there were some differences in the variation trend of annual effective dose among different occupational categories, the overall trend was decreasing year by year. The average annual effective dose of nuclear medicine and interventional radiology was relatively high, and the difference was not statistically significant (P>0.05). The average annual effective dose of industrial flaw detection and accelerator operator in industrial application was relatively higher, and the difference was not statistically significant (P>0.05). There were statistically significant differences in the per capita effective doses of different occupational categories of medical application in different years (F=8.892-682.852, P<0.001). Among them, the per capita annual effective doses of five occupational categories of medical application were the highest in 2016, and the per capita annual effective doses of interventional radiology were the highest in 2018 and 2019. Conclusions The per capita annual effective dose of radiation staff in Chongqing has decreased year by year from 2016 to 2020, and remained at a relatively low level that meets national standards. The monitoring results suggest that the radiation staff of nuclear medicine, interventional radiology, industrial non-destructive testing and accelerator operator should give special attention to protect their occupational health and safety.

ABSTRACT

In 2003, the responsibility of radiation source safety supervision was redivided and transferred from the health department to the environmental protection department. After the promulgation and implementation of the Rule on the Administration of Radiodiagnosis and Radiotherapy in 2006, due to the incorporation of a large number of medical practice supervision rules, the radiation health supervision of medical institutions has evolved into the radiodiagnosis and radiotherapy supervision. In the past 16 years, radiation, occupational health supervision, and medical practice supervision have not been well integrated, and there are many legal problems in radiodiagnosis and radiotherapy supervision. The regulation rules of environmental protection, metrology, and other relevant departments on radiation sources and radiation devices have been constantly changing, resulting in overly complex regulatory requirements on radiodiagnosis and radiotherapy, which increases the burden of medical institutions. This paper reviews the historical evolution and legal system changes of radiodiagnosis and radiotherapy supervision, analyzes the problems and challenges faced by the relevant regulatory departments, and proposes ways to improve the supervision system.

ABSTRACT

Objective To analyze the administrative penalties for radiation diagnosis and treatment in Xuzhou, China, 2018—2021, investigate the key illegal behaviors in radiation diagnosis and treatment, and provide a basis for effective supervision with targeted measures. Methods A total of 142 cases of administrative penalties were analyzed for the year of the case, the undertaker, the basic information of the subjects, the amount of penalty, and the specific illegal facts. Results From 2018 to 2021, the case number of administrative penalties for radiation diagnosis and treatment in Xuzhou was relatively stable. The cases were mainly undertaken by the municipal health supervision institution, while the number of cases handled by basic-level health supervision institutions was increasing. The administrative penalties were mainly imposed on persons and primary or non-classified medical institutions. Common illegal behaviors included occupational health management of radiation workers and personal protection during medical exposure. Conclusion We should focus on strengthening the publicity and supervision of personal protection in occupational health management and medical exposure, continue to improve the law enforcement ability of basic-level radiation health supervisors, develop a standardized management system for radiation diagnosis and treatment, innovate the supervision mode and radiation hazard notification mode, promote the “Internet + ” informatization supervision, and recommend to improve and revise radiation-related laws and regulations.

ABSTRACT

Background Most of the studies on grading and classification of occupational health compre-hensive risks for specific employers still remain in the establishment and description of methodology, and practical application studies are rarely reported. Objective To explore the application of an occupational disease hazards comprehensive risk assessment method issued by the National Disease Control and Prevention Bureau in conjunction with the National Health Commission and a self-developed occupational health grading and classification method in petroleum refining enterprises, and to provide practical experience for the implementation of differentiated law enforcement by relevant regulatory authorities. Methods Two occupational health grading and classification methods were practiced in three petroleum refining enterprises in Guangdong Province. The occupational hazards comprehensive risk assessment method was provided by the Notice on Carrying out Pilot Work of Occupational Health Classification Supervision and Law Enforcement of the National Disease Control and Prevention Bureau and the National Health Commission. The principle was to derive the occupational health risk level according to nature of occupational hazards, exposure level, and number of workers exposed to them in an employer, and then to classify them into Class A, Class B, and Class C by combining with local occupational health management status level. The occupational health grading and classification method was self-developed according to available domestic and foreign occupational health risk assessment methods. Its principle was to calculate the risk level of each workstation in an employer based on published carcinogenicity classification or LD50/LC50 of chemical toxicants, level of noise, exposure parameters such as exposure level and exposure frequency, estimate the comprehensive risk Ro of the target employer by the Romanian comprehensive risk calculation method, and then calculate a comprehensive risk Ro' weighted by the occupational health management index of the target employer and classify it into class A, class B, and class C. Finally, assessment results, scope of application, inquired indicators, advantages,disadvantages and professional competence requirements of the two grading and classification methods were compared. Results The occupational hazards that were evaluated in three enterprises in this study were benzene, toluene, xylene, gasoline, hydrogen sulfide, and noise. The exposure levels of benzene, toluene, xylene, and gasoline were all below 10% OEL (occupational exposure limit), and hydrogen sulfide and noise were disqualified. Occupational hazards such as benzene and hydrogen sulfide were serious occupational hazards in the three enterprises, and the number of workers exposed was 461, 912, and 224, respectively; the HRs (hazard ratings) of benzene, toluene, xylene, gasoline, hydrogen sulfide, and noise were level 5, 3, 2, 3, 5, and 3 respectively. The occupational health management status of the three enterprises was graded as B, A, and B, respectively by the occupational disease hazards comprehensive risk assessment method. The occupational health management index of the three enterprises was graded as B, A, and A, respectively by the occupational health grading and classification method. The comprehensive risk assessment results showed that two enterprises classified into as the highest class C and one into class B by the occupational disease hazards comprehensive risk assessment method, while all three enterprises were classified into class B by the occupational health grading and classification method. Conclusion The two grading and classification methods are not consistent in the evaluation results of petroleum refining enterprises, and there are differences in the evaluation scope, indicators to be collected, and professionalism. It is recommended that occupational health regulators should fully consider the advantages and disadvantages of the two methods and choose the appropriate assessment method according to the actual regulatory purpose.

ABSTRACT

In the face of a serious occupational disease prevention and control situation, the lack of efficient supervision and regulation methods has become a prominent contradiction. This paper reviewed the progress and application of occupational health grading and classification supervision methods for employers in China in recent years, and analyzed the characteristics and limitations of selected methods. The grading and classification supervision methods have gradually developed from considering only inherent health risk of occupational disease hazards, to considering both inherent attributes of occupational disease hazards and exposure duration. Some regional authorities introduced trial measures for grading and classification of occupational health supervision by embracing exposure level of an occupational disease hazard of interest, associated outcome severity, and occupational health management situation of targeted employers. In October 2022, the National Disease Control and Prevention Bureau issued the Notice on Carrying out Pilot Work of Occupational Health Classification Supervision and Law Enforcement, which further promoted the establishment of a differentiated occupational health supervision model. Grading and classification of occupational health supervision can improve the supervision efficiency of public health administration departments and promote employers to fulfill the main responsibility of occupational disease prevention and control.

@#Peer-assisted learning (PAL) is a potential approach for clinical education that can reduce the burden of clinical supervision and enhance learning. This study aims to identify opportunities for PAL through investigating how speech language therapy students perceive and already participate in PAL within a range of clinical practicum settings. The Speech Sciences students across all years at one institution were invited to participate in a cross-sectional survey that was adapted and revised from Tai et al. (2014). Speech Sciences students reported they applied PAL as part of their learning strategy in their clinical practicums, but still relied on supervisors as the main source of. PAL occurred more frequently in contexts where students already had some clinical experience and spent substantial amounts of time together. Students agreed that PAL enhanced their learning and emphasized a few advantages of PAL, such as having the opportunity to share their ideas, experiences, and knowledge, and providing a positive learning environment without pressure. Confidence to provide information or feedback to peers was cited as a shortcoming in PAL. PAL is a viable teaching approach that can be used in speech-language therapy clinical education program to reduce the supervisory burden. The findings from our study show that PAL is largely self-initiated among speech-language therapy students. However, PAL must be tailored to suit different clinical education year levels and clinical settings to benefit. Case discussion may be one area where scaffolded PAL activities could represent a feasible first step to increasing PAL.
